To register for a Sturgis Library CLAMS card, print this form, fill it out, and bring it to the library with a piece of valid identification showing your current address.
If you are a Massachusetts resident, you can apply for a temporary CLAMS e-card by filling out this online form and following the instructions. It is valid for 6 months.
Please be aware that we do not issue library cards to those living out of state unless you visit Sturgis Library in person to borrow materials during your stay on Cape Cod.
